How to subscribe to the NHI |

| The insured |
- The insured may enroll to the NHI through the following group insurance applicants according to their category and status.
- Category 1 and Category 2: They shall enroll to the NHI through the agency, school, business, institution, employer or group for which they are working; except for employees of Ministry of National Defense (MND) that the group insurance applicant shall be appointed by the MND.
- Category 3: They shall enroll to the NHI through the farmer’s association, irrigation association or fishermen’s association of which they are a member or which located within the administrative district of their registered household address.
- Category 4: They shall enroll to the NHI through the group insurance applicant appointed by the Ministry of the Interior (MOI) or MND.
- Category 5 and Category 6: They shall enroll to the NHI through the township (village, city or district) office to which their household is registered. The insured accommodated in public or private social welfare service institutions may enroll to the NHI through respective social welfare service institutions.
- Category 6-2 and their dependants: They may enroll to the NHI through the group insurance applicant of the insured of other categories living together with them, provided prior approval from the group insurance applicant shall be obtained, and the premium shall be calculated according to the insured of a community insurance unit.
- Personnel of public schools and civil servants, or employees of public or private businesses or organizations, or employee of a particular employer may enroll to the NHI under Category 6-2 through the township (village, city or district) office to which their household is registered when they are unemployed after retirement.
- Retired people meeting the Category 6-2 requirements may enroll to the NHI through the original group insurance applicant (employer), provided prior approval from the group insurance applicant must be obtained, and the premium shall be calculated according to the insured of a community insurance unit.
- Insured of a community insurance unit may enroll to the NHI through the religious organization or local religious group where their household is registered.
- Personnel of public schools or employees of public or private businesses or organizations, or employees of a particular employer may enroll to the NHI through the original group insurance applicant (employer) at the original insurance amount when they are on a leave of absence (LoA), provided prior approval from the group insurance applicant shall be obtained.
- Insured of special occupations or self-employed professionals (without hiring any employee) may enroll to the NHI through respective occupational unions.
- The insured under Category 6 receiving training from a legally registered vocational training institution or examination training institution shall enroll to the NHI through such institutions, and the premium shall be calculated according to the insured of a community unit (Category 6).

| Enrollment requirements |
◎ ROC citizens meeting any of the following requirements shall enroll to
the NHI.
- ROC citizens who have household registration in Taiwan for 4 months.
- ROC citizens who have household registration in Taiwan and a regular employer, or who are dependents of servicemen and dependents of an unemployed person. Newborns whose birth is registered in Taiwan and who meet the dependent requirements of the insured.
◎ Expanded coverage and loosened application requirements
(implemented as from 17 July 1999)
- Aliens and overseas Chinese from Hong Kong, Macau and China holding a valid Alien Resident Certificate (ARC) and have been living in Taiwan for 4 months are required to enroll to the NHI according to the status since the fifth month from obtaining the ARC; except for those who have a regular employer that they shall enroll to the NHI through their employers from the day of inauguration (Article 10). Aliens and overseas Chinese from Hong Kong, Macau and China making a make-up enrollment for the NHI shall pay the premium as from the day when they are valid for the NHI, regardless of their status, i.e. either the insured or a dependent. Alien spouse of a ROC citizen, including citizens of Hong Kong, Macau and China, shall enroll to the NHI as from 1 July 1998.
- Aliens and overseas Chinese from Hong Kong, Macau and China enrolling to the NHI under Category 6 shall enroll to the NHI through the township (village, city or district) office to which their household is registered.
- ROC citizens who have enrolled to the NHI and have household registration in Taiwan shall enroll to the NHI, regardless of the 4-month household registration requirement.
- Contract employees of military establishments are required enroll to the NHI through the Rear Echelon Administration, Combined Service Forces General Headquarters, as from the announced effective day of this amendment.
- Those who are under protection custody are required to enroll to the NHI.
- Chinese spouses holding a reunion visa shall enroll to the NHI as the dependent of their Taiwanese spouses after staying in Taiwan for 4 consecutive months.
◎ Military officers have been covered in the NHI scheme as from 1
February 2001 and become valid NHI insured.
- Officers serving the voluntary military service shall enroll to the NHI through a group insurance applicant formed by the Rear Echelon Administration (REA), Combined Service Forces General Headquarters (CSFGH). This group insurance applicant shall be within the jurisdiction of the Taipei Branch of the Bureau of National Health Insurance.
- Officers who are regular servicemen of 20 years of age and serving the compulsory military service are qualified for Category 4-1. They shall enroll to the NHI through REA together with cadets of the military academies, homeless military dependents, and dependents of deceased military officers; and their premium shall be paid in full by the MND.
- Draftees serving a substitute service are qualified for the Category 4-2. They shall enroll to the NHI through the Central Taiwan Office of MOI and their premiums shall be paid in full by MOI.
- Military training officers of the Ministry of Education (MOE) teaching at schools shall apply to a local BNHI branch for forming a group insurance applicant in the name of the Military Training Office of the school to enroll to the NHI.
- Dependents of military officers serving the voluntary service enrolling to the NHI under Category 4 through REA shall change their status to Category 1 and enroll to the NHI through the new group insurance applicant formed by REA.
- Dependents of military officers serving the compulsory military service shall enroll to the NHI according to their status and shall not enroll to the NHI as a military dependent because the compulsory military service is a temporal duty.
- Employers shall check the “transfer out” item instead of the “withdrawal” item, and specify the reason: military service in the NHI Withdrawal Application Form for employees or their dependents who are conscripted as from 1 February 2001.
◎Disqualification of NHI
The NHI enrollment of the following people will be rejected or withdrawn even they have enrolled to the NHI.
- The insured is serving his term in the prison or detention center, or under security penalty or disciplinary education, except for a term under 2 months or protection custody.
- People who have been missing for 6 months.
- Those who are disqualified according to Article 10 of the National Health Insurance Act.
- Dead people.
- As from 13 September 2002, Chinese citizens holding an ARC or a reunion visa issued by the ROC government and have enrolled to the NHI shall withdraw from the NHI as from the day of departure if they have left Taiwan for more than 120 days before the next entry

| Group insurance applicant |

◎ Insured categories in NHI scheme
According to the NHI scheme, the category or status of the insured is determined by the employment status of the insured. Categories of the insured are defined in the following table. The insured may find out what category or status they may use when enrolling to the NHI. |

| Underwriting form completion |
◎Notice for underwriting form completion
- Complete the group insurance applicant code (ID) inn all underwriting forms and affix to all forms the seal of the group applicant unit and the seal of statutory representative.
- When the insured and his dependent(s) wish to enroll to the NHI at the same time, their basic data shall be completed individually in the NHI Enrollment Application Form. When the insured wishes to withdraw from the NHI, complete the basic data of the insured in the form, and the system will automatically display the basic data of his dependent(s). If a dependent of the insured wishes to transfer out from the insured, complete the data of the insured and that dependent in the same row and remark that dependent with a ‘v’.
- Check exactly the “insured” or “dependent” columns exactly in the NHI Enrollment Application Form and NHI Withdrawal Application Form to prevent data input errors.
- Specify exactly the “cause” and “date” of enrollment and withdrawal to facilitate the validation of effective day of enrollment and withdrawal and to prevent premium calculation errors. (Complete the exact date of event occurrence in the date column, e.g. for NHI enrollment of new employees, specify “inauguration” in the Cause of Enrollment and complete the “date of inauguration” in the Date of Enrollment; for NHI enrollment of dependents, specify “transfer in upon the inauguration of the insured” in the Cause of Enrollment and complete the “date of inauguration of the insured” in the Date of Enrollment; for NHI enrollment of a newborn, specify “newborn” in the in the Cause of Enrollment and complete the “date of birth” in the Date of Enrollment.
- A declaration (statement) specifying the reasons(s) that the dependent is unable to enroll to the NHI as the dependent of his closer family member shall be submitted in applications for enrollment of dependents who are lineal descendents by blood but of different levels of kinship.
- Specify the cause of enrollment for the enrollment of dependents of 20 or older who are lineal descendents by blood. (See instructions in the NHI Enrollment Application Form for details, the cause codes include S, P, A, H and G.)
◎Note
Foreign workers hired by a Cat. 1 group insurance applicant must not enroll to the NHI until they have received an ARC issued by the ROC government. The applicant shall also submit the photocopy of the working permit for the work they are engaging issued by the authorities and a list of foreign workers. |

| Underwriting check |
The National Health Insurance (NHI) is a mutual aid and risk-sharing social security system. For the reason of solidarity, it is necessary for all citizens to enroll to the NHI and pay the premium in order to enjoy appropriate medical benefits. Therefore, how to enforce the compulsory enrollment to the NHI of all citizens and to eliminate violations of the NHI to ensure the sustainable operation of the scheme has become the primary goal of underwriting check and the responsibility not to be shirked between you and me.
◎ Type of cases to be checked
The underwriting check is normally conducted spontaneously by the BNHI or upon the report of citizens. Cases to be checked generally classified into the following four types.
- Cases involving employers who did not form a group insurance unit or enroll to the NHI for their employees.
- Cases involving enrollment with untrue categories. For example, professionals or employers did not enroll to the NHI under Category 1 but under Categories 2, 3 or 6 through an occupational union, farmer’s association or district/city/township office or as the dependant of the insured, or enroll to the NHI at a lower insurance amount.
- Cases involving enrollment at a lower salary for employees or at a lower insurance amount.
- Cases involving false report of not enrolled employees.
◎ Have you subscribed to the NHI by the rules?
- An information exchange channel with the household registration database has been established to locate citizens who have not subscribed to the NHI by comparing the household registration data with the data of the insured. Aggressive guidance will be given to citizens who have not subscribed to the NHI to realize the all-out participation target.
- A cross examination with the BLI data has been conducted to find out if group insurance applicants have subscribed to the NHI for all employees by the rules, or if they have subscribed to the NHI for them at a lower insurance amount.
- A data matching with the Financial Data Center (FDC) has been conducted to find out group insurance applicants who did not subscribe to the NHI for their employees or who have subscribed to the NHI for them at a lower insurance amount in order to provide them with guidance.
◎ Grievance channels
- In detecting any of the following items in the group insurance unit, please file a grievance to the NHIED over the phone, in writing or in person (please specify the your name, ID card number, date of birth, contact address and phone number). We shall handle your grievance as soon as possible, and all your information is classified.
- The employer did not form a group insurance unit.
- The employer refuses to subscribe to the NHI for the employees and their dependants.
- The employer subscribes to the NHI for the employees under a different category.
- The insurance amount in the subscription is lower.
- The employer collects premium illegally.
- The employer collects charges illegally.
- Should the insured or group insurance applicant have doubts regarding the settlement any grievance or item concerning other rights and benefits, please contact the NHIED for details. Should you not agree to the decisions on a grievance or approvals of the BNHI or NHIED, you may file a grievance in writing to the BNHI or NHIED (please specify your name, ID card number, date of birth and contact address and phone number), we will handle and reply to your grievance reasonably.
◎ Penalty
- If a group insurance applicant fails to subscribe to this Insurance [pursuant to Article 16] for the insured or their dependents, it shall be punished with an amount equivalent to two times of the payable premiums in addition to the unpaid premium (Paragraph 1, Article 69, National Insurance Act).
- If a group insurance applicant refuses to pay by law the premium for the insured and his dependents [pursuant to Article 30], and the premium is paid by the insured, in addition to returning the insured premium paid, the group insurance applicant shall be punished with an amount equivalent to two times of the payable premiums (Paragraph 3, Article 69, National Insurance Act).
- If a citizen refuses to subscribe to this Insurance pursuant to this Act, he shall face a fine of no less than three thousand but not more than fifteen thousand New Taiwan Dollars and shall subscribe to this Insurance retrospectively from the date when he is eligible for the subscription. The benefits shall be suspended before the fine and premium are paid in full (Article 69-1, National Health Insurance Act).
In the event that a group insurance applicant subscribes to this Insurance for the employees under Category 1 at an amount lower than their insurance amounts, a fine in the amount of two to four times of the payment of different premium shall be imposed in addition to the payment of premium differential (Article 73, National Health Insurance Act). |

| Formation, alteration and revocation of group insurance applicants |
◎ Application for formation
Applicants shall prepare TWO sets of the following forms and documents and post them to the HNIEB.
- The Group Insurance Formation Application Form (Form A) and Insured Transfer In Table (Table D).
- Submittals
- Photocopy of both sides of the ROC Citizen ID Card of the statutory representative.
- Photocopy of both sides of the Business Registration Certificate (if there is a change of the statutory representative, please also submit the photocopy of the registration change record).
- Applicants may submit the photocopy of the Uniform Invoice Purchasing Certificate or other taxation information when both documents in 1 and 2 have not yet been issued, provided the date of business inauguration and the scope of businesses shall be specified.
- Photocopy of the practicing license (physician, attorney-at-law, architect, CAP, land administration agent and other professionals running their own businesses).
- Photocopy of the certificate of qualification issued by the Examination Yuan (physician, attorney-at-law, architect, CAP, land administration agent and other professionals running their own businesses).
- Photocopy of the Notice of Assignment of Government Uniform Invoice Number for Taxpayers issued by the National Tax Administration (physician, attorney-at-law, architect, CAP, land administration agent and other professionals who have opened an account at the NTA shall submit this notice).
- Photocopy of the Civil Organization Registration Certificate and Certificate of Elect.
- Other documents approved by the authorities
- Additional documents shall be submitted if the insurance amount of the statutory representative is under $131,700.
- The statutory representative shall submit a concise list for report by evidence or reduction of insurance amount.
- Documents and procedures required for forming a new NHI group insurance applicant.
◎ Application for formation
A group insurance applicant shall submit the TWO sets of the following documents to the NHIBE by post within 15 days from the approval for termination, suspension, dissolution or registration cancellation of a business by the authorities.
- Two photocopies of the approval documents for termination, suspension, dissolution or registration cancellation of a business by the authorities.
- Two copies of the NHI Insured Withdrawal Application Form with exact date of discharge of the insured (send a copy to the discharged employees for transferring in to the new employer).
- Notice for application for withdrawal and rear personnel of the group insurance applicant.
- The group insurance applicant shall complete the transfer-out procedures for all discharged employees before obtaining the approval for termination, suspension, dissolution or registration cancellation of a business by the authorities. The employer shall subscribe to the NHI through the original group insurance applicant until the approval is obtained, except the subscription has been made through another group insurance applicant in the name of statutory representative or employee.
- If there are rear personnel after application for business termination is approved, the rear personnel.
◎ Applications for basic data alterations
A group insurance applicant shall apply for basic data alteration (certificate address, mailing address, name and/or statutory representative) according to the following procedure.
- Forms to be completed: NHI Group Insurance Applicant Basic Data Alteration Application Form (specify only the updated date in items to be altered; skip items without data alteration; and affix the seals of the group insurance applicant and statutory representative).
- Submittals (photocopy)
- Government agencies, public schools and state businesses: group insurance applicants of such status shall submit the documents of approval for basic data alteration, except mailing address; and both sides of the ROC Citizen ID Card of the new statuary representative if it is an alteration of the statuary representative.
- General private businesses and organizations:
- Application for mailing address alteration: No need of submittals.
- Application for registration address alteration: group insurance applicants shall submit the new Business Registration Certificate issued by the authorities after approval (MOEA, department/bureau of economic affairs of local governments, or the competent authorities of respective businesses), and photocopy of documents of different types of group insurance applicants:
a.
company: approval letter, business registration certificate or business license issued by the competent authorities;
b.
business: business registration certificate or business
c.
license;factory: factory license;
d.
group insurance applicants without a business registration certificate or business license (including civil organizations
of occupations, self-practicing professionals and technologists, e.g. lawyers, CPA etc): approval documents for registration alteration, or notice for registration alteration issued by the tax administration office, or establishment or registration documents issued by
respective competent authorities; and
e.
group insurance applicants of local representatives of
foreign companies: approval documents issued by the
MOEA.
- Application for statutory representative alteration: In addition to submitting the said documents, applicants shall submit the photocopy of both sides of the ROC Citizen ID Card of the new statutory representative. However, the change in the statutory representative of group insurance applicants of self-practicing professionals/technologists and local representatives of foreign companies shall not be practiced in the form of statutory representative alteration but revocation of the old group insurance applicant and formation of a new group insurance applicant.
- Application for group insurance applicant name alteration
a.
Applicants without altering the Uniformed Business Tax
Code: submit documents as shown above.
b.
Applicants whose the Uniformed Business Tax Code has been altered (e.g. a company changed into a business, or the vice versa): the original group insurance applicant shall be revoked, and the new unit shall form a new group insurance applicant.
- Applicants shall submit the required documents for each item to be altered in applications for alteration of two or more items in the basic data.
When a military group insurance applicant applies for altering the name and address of the unit, it shall submit the NHI Group Insurance Applicant Basic Data Alteration Application Form and documents from the MND. |

| Common forms for LI and NHI |
Notice for using common forms for Labor Insurance and NHI
- When completing the common forms for the Labor Insurance (LI) and National Health Insurance (NHI) for reporting to the Bureau of Labor Insurance (BLI) and Bureau of National Health Insurance (BNHI) at the same time, don’t forget to complete two sets of forms before mailing it the Eastern Region Branch of the BNHI (NHIED) at 36 Suanyuan Road, Hualien City. Please specify on the right top corner of the forms LI or NHI, so that we can redirect the forms to the correct offices.
- Before mailing these forms to us, always make a photocopy of all forms and put the forms for BLI outside (because BLI considers the date of acceptance according to the date appears in the post stamp). Then, send everything to the NHIED by registered (recorded) mail. Make sure to attach the receipt to the photocopy of the forms to facilitate future reference.
- Some think that it will be easier to send these forms to the BLI and BNHI individually. But please don’t do so. If the forms are the common forms for the BLI and BNHI, we will make another copy and send the original to the BLI there is only one copy in the envelope. BLI in Taipei will do the same thing and send us a copy when receiving the forms you directly mailed to them. In this case, the NHIED will receive, accept and review the forms of the same case twice. Because of this misunderstanding, this will cause a waste of postage for recorded mails by the applicants; the photocopy, postage, labor and other managerial costs of the BLI and BNHI. Therefore, please do not send the common BI and NHI forms to the BLI and BNHI individually!
- In applications for only NHI data alterations, applicants will simply need to cross out the Labor Insurance XXX Application Form in the forms and affix the seal of the responsible person next to it.
